Fees

Lab Fees:

Meridian Women's Health contracts out their lab to Northwest Hospital Clinical Laboratory and Cellnetix for Pathology services. If you have questions regarding your lab or pathology bill, please contact them directly at Northwest Clinical Laboratory (206-368-6440) or Cellnetix Pathology (1-800-374-4045).

No Show Fee:

Due to the large volume of patients in our practice we require at least 24-hour notification of a cancellation. This enables us to offer your appointment time to another patient in need. Our office is available to take cancellation calls from 8:00am to 4:00pm Monday - Thursday and Friday 8:00am-12:00pm. Please note that calls made to the answering service after hours will be considered less than a 24-hour notification. We do charge a $75.00 late cancellation/no show fee that must be paid in full before another appointment will be scheduled.

Medical Records Fees:

Meridian Women's Health provides copies of patient's records upon written request. As a courtesy to our patients and other physicians, Meridian Women's Health provides copies of records to physicians' offices free of charge. If the patient should request a copy of their records there is a nominal copying fee. You may call our office to receive a copy of these fees 206-368-6644.

Parking Fees:

UW Medicine Northwest Outpatient Medical Center does charge for parking. Please click here to see the current rates.

Other Fees:

Most insurance plans today have co-payments for physician visits. Payment is required before each visit. Please contact our insurance coordinator at 206-368-6644 if you need further information. We also encourage you to call Northwest Hospital & Medical Center (206-368-6440) to confirm they are participating with your insurance company, before your surgery or delivery.
